HOW TO CONTRIBUTE:

1. Download the I-330 Tax Form from the S.C. Department of Revenue Web site www.sctax.org. Or, click here for a direct link to download the form.

2. Enter the amount of your donation on Line 6 of S.C. Tax Form I-330.

3. Then, enter the same amount on Line 13 of S.C. Form 1040A (Line 28 of S.C. Form 1040).

4. Attach Form I-330 to your tax filing.
