Family Connects is a nurse-based approach to supporting newborns and their families through nurse home visits, connecting families to services in the community, and supporting families with an individual approach to care.

Who it Serves

All individuals who deliver at a participating birthing center or hospital are eligible to receive services.

How It Works

Families are offered a free home visit by a nurse, who conducts a maternal and child health screening and connects parents to resources in their community.
